On-site registration at the venue

Please arrive early if you wish to register on-site. Please note that it cannot be guaranteed that on-site registrants will receive all congress documentation, and that availability of hotel accommodation at that time might be limited.


Payment Details

Payments must be submitted at the same time as the Registration Form by either Bank transfer or Credit Card.

Forms sent without payment proof or Credit Card authorizations will not be considered. Please note that cheques are not accepted.

All bank transfers must be sent in EURO € funds to:  :  Garanti Bank Elmadag Branch Code : 234 – Account No. 9088762   IBAN NO TR42 0006 2000 2340 0009 0887 62- Please specify "EACID 2009" and your name on the bank transfer. All bank fees must be borne by the remitter.

Accepted Credit Cards are Visa, USDocard and Mastercard. The card number, expiry date and name of the cardholder should be stated in the appropriate space on the registration form.

Registration will be confirmed by either email or fax ONLY after receipt of the full registration fee (including the bank fees). A receipt for the payment will be sent by surface post. The receipt will be issued and sent to the name and address indicated in the form.

In case of cancellation, a written notification should be sent to the Congress Secretariat. All refunds will be made after the Congress.

If you are unable to attend, the name and address of a substitute must be notified in writing.

• If received before , August 20, 2009 the participant fee will be refunded.
• On or after , August 20, 2009 a cancellation fee corresponding to the entire participant fee will be charged.
